Our mHealth App

In 2023, we received a Phase I Small Business Innovation Research (SBIR) grant from the National Institutes of Health (NIH) where we developed the PATH2Caregiving mHealth app to assess and address the needs of family caregivers. The PATH2Caregiving mHealth app uses the validated Preparedness Assessment for the Transition Home (PATH)© assessment and Catalogue of Interventions© to deliver algorithmically tailored and engaging resources and education to family caregivers of individuals with a disabling condition to prepare them for their role and decrease the adverse health effects of caregiving.

The digital mHealth application of the PATH2Caregiving™

​

  • Is designed to provide tailored information to caregivers of patients who are hospitalized with a disabling illness.

  • Delivers educational content and community resources based on the patient’s diagnosis 

  • Is tailored to the individual needs of the family caregiver based on the PATH© assessment.

 

The Objectives

​

  • Improve family caregiver engagement in the transition planning process through the use of a user-friendly mHealth app to deliver tailored education and resource materials

  • Experience the associated improvement in caregiver well-being 

  • Improve care experience and associated enhanced reputation as caregivers share their positive experience preparing for their role

  • Enhance efficiency of the discharge/transition planning process through automation of the response to the PATH© caregiver assessment

  • Reduce paper and plastic by eliminating hard copy delivery of materials

  • Allow multiple caregivers to have access to the education and resources

 

Advantages of the Digital Path2caregiving Program

  • Algorithmically-delivered content tailored to the organization’s population and geographic resources

  • In addition to a wealth of built-in education and resource materials hosted in the app, the facility representative can easily upload additional resources (pdf, video, audio, web links) as needed. No re-formatting is necessary.

  • Organization has full control over adding/deleting organization-specific content and adding and deleting users. Thus, there are no fees associated with these activities

  • User interface and language optimized for health literacy (readability & understanding)

  • Tested in clinical nursing, case management, and family caregiver population with demonstrated usability and acceptability

  • The primary family caregiver can easily add access for other family members on the platform/mHealth app

  • Saves paper and plastic and supports corporate sustainability efforts and the United Nations Sustainable Development Goals

The next step of our journey is to evaluate the feasibility of workflow integration of the PATH2Caregiving app in a real-world inpatient rehabilitation facility (IRF) setting. Concurrently, we will apply for Phase II NIH SBIR funding to further advance the product for integration with the electronic health record, language translation, and utilization of artificial intelligence.

This is a subscription-based mHealth app available to healthcare facilities for individual caregiver use.
