The PATH2Caregiving Program is different.

The PATH2Caregiving Program combines the path assessment tool (Preparedness For The Transition Home) and a tailored caregiver intervention program.

 

PATH is a freely accessible, evidence-based assessment tool that is designed to assess caregivers' preparedness to transition patients home. Originally, created to assist stroke survivors and their family caregivers, it has since been implemented with a cross-section of caregivers of patients with other conditions to assist with the transition home.

 

Created by Michelle Camicia, Ph.D. RN CRRN CCM NEA-BC FARN FAHA FAAN who has been a leader in rehabilitation nursing for over 30 years and Barbara Lutz, Ph.D. RN CRRN PHNA-BC FAHA FAAN who has conducted rehabilitation nursing research related to comprehensive stroke care for more than 20 years.

In their extensive experience and research, they noticed a 'gap' in the care continuum and the need for a systematic approach for a patient's care team to engage and help caregivers prepare for their role. Understanding that care facilities and case management might 'already' be assessing the caregiver, PATH2Caregiving is confident that they have created a system that is BETTER! It is patient/family-centered, improves caregiver health, and delivers a positive impact.

 

The PATH and associated program was deemed "exemplary by the ANCC Magnet Recognition Progam™ for meeting the specific and individual social needs of new family caregivers.

Who We Serve

CAREGIVERS

We ultimately serve the Caregiver who is responsible for the continued care of the patient. When we build confident caregivers it helps lessen the chances of the patient being readmitted into a facility.

CASE MANAGERS & SOCIAL WORKERS

We don't want to give case managers and social workers' 'one more thing' to do but to streamline the process with a systematic approach that will fully assess and address the needs of the caregiver.

MEDICAL AND REHAB FACILITIES

It is a sad truth that many patients and caregivers have felt "abandoned" by their medical and rehab facilities. Let's change that! With our PATH2Caregiving Program, we can optimize the patients' transition home with prepared caregivers.
